Preface:
Cough is an explosive expiration, which provides a means of clearing the tracheobronchial tree of secretions and/or foreign bodies. Though not a disease in itself cough is frequently present in diseases of respirtory System. Cough may also be present as reflex symptom of uterine disorders or of nervousness.
The character of cough does indicate the anatomical site of cough eg. barking cough shows epiglottic involvement, the brassy character shows tracheal or major airway obstruction. The time of occurance may suggest a specific cause eg. cough occurring selectively at night suggests Congestive Heart Failure or cough related to meals suggests hiatus hernia or oesophagial diverticulum etc. The character of sputum too may point towards diagnosis.
The cough may be dry or productive, painful, short or paroxysmal, explosive or with stridor. The character of cough sometimes may lead to the diagnosis, but whatever the diagnosis, appropriate remedy can be found by knowing the character of cough by careful history taking to make the symptom as much complete as possible and to match these with those of medicine.
